1. Enter the name of the form


2. If 'Generate Submission ID' is checked a unique identifier will be created with each submission of the form


3. If 'Require Member Login' is checked it means users cannot complete and submit the form unless they have and account with the website and are logged in


4. Waiting List Container: Note: This field is specific to the Registration module and is not applicable when building a basic form


5. User Notify Field: This field must have a value selected in order for the notify templates in field 8 to send


6. Emails From Field: Determines the email, or name of the from field on email notifications


7. Tax override is only applicable to websites that have the Purchase Item Module enabled


8. User Notify Template is used to send an email to users after the form is submitted. Select an existing template by clicking on the dropdown, edit an existing by licking on the blue pencil icon, or create a new one by clicking on the green plus icon


9. User Invoice Template is used to send an email to users after they have made a payment. Select an existing template by clicking on the dropdown, edit an existing by licking on the blue pencil icon, or create a new one by clicking on the green plus icon


10. Admin Notify Template is used to send an email to an administrator(s) after a user has submitted a form. Select an existing template by clicking on the dropdown, edit an existing by licking on the blue pencil icon, or create a new one by clicking on the green plus icon


11. Admin Invoice Template is used to send an email to an administrator(s) after a user has made a payment. Select an existing template by clicking on the dropdown, edit an existing by licking on the blue pencil icon, or create a new one by clicking on the green plus icon
