On the questions tab of the form builder click and drag different types of fields into the form space. Once the filed is dragged over click on the edit icon (blue pencil) to enter details.

There are twelve types of fields that are detailed below


Text Content

This field will be for messages and statements on the form. It will not be filled out by users.


Text Field

Text field will prompt users to answer a question in plain text format


1. The Question Label is not displayed on the front end and can be a title that makes sense for the question


2. Prompt Type can be in plain text (defaulted) or HTML. It is generally best to use Plain Text.


3. Question Prompt. This is the question label on the front end of the form.


4. Field Width describes the size of the box text can be entered into.


5. Choose between preloaded merge tags, or create a custom merge tag for the field.   

Merge Tags
Merge Tags are used to include form field submissions into the notification email that goes out to users and/or administrators.

To setup a merge tag, first, edit the question you'd like to 'merge' into the email notification and set its Merge Tag field; for this example we'll use MYFIELD.

The entry into the question settings popup window does not need the asterisk or lines.
Then, in the email template, simply type *|MYFIELD|* wherever you'd like the value of that field submission to appear.

The merge tag entry into the email template does need the asterisk and lines.


6. If export by default is selected the information in this field will automatically be pulled when submission details are exported to an excel spreadsheet.


7. If Is Required is checked users will not be able to submit the form without filling out the field


8. Default Text can be used to provide an example of the information provided.




Text Area

Text area works the same as text field. The difference is there is more space for text


Email

preset field for email addresses. Set merge tag and required checkbox if applicable.


Phone Number

preset field for phone numbers. Set merge tag and required checkbox if applicable.


Full Name

preset field for first and last name. Set merge tag and required checkbox if applicable.


Checkbox

Use the checkbox field when there are multiple options that can be selected.

7. Alignment determines whether the options in the checklist will display on separate lines (vertical), or all on one line (horizontal)


8. Enter in the the different options that can be selected. To delete an option click on the red icon to the right. 

    To add an option click on the Add option button. 

    To sort options alphabetically or numerically click on the Sort Options Button



Radio Button

The radio button provides options that users can select one or the other, such as yes, or no.

Follow the same steps as checkbox above to complete this question.


Dropdown List

The function of a dropdown list is similar to a radio button in the sense that only one option can be chosen, but it is better for a larger number

Address

preset field for phone numbers. Set merge tag and required checkbox if applicable.


Date

Preset field for date. Enter question prompt and merge tags if applicable


File Upload

File upload allows users to insert a file to the form.
